waswasa (evil thoughts)

Assalaam u alaikum I have problems regarding Islam and I severely suffer from waswasa. I was wondering if you could help me. I have a problem with making intentions for salaah and wudhu and I am not sure how to make them. I know that in intention is meant to come from the heart but what does that mean? how does one make an intention in the heart? I also have a problem while I am making my intention, pictures of swear words appear in my mind. I am not sure then whether my intention is valid so I restart my intention. Sometimes this could go on for around 15 mins.Please advise me what to do here Jazakallah for your time.

1. Avale kitsoz pendant namaaz. 2. Couma pou faire si wouzou fine casser dans namaaz?

Ass. 1.Eski si nourriture ou bien miswaak meme fine tasse entre nou les dents et qui nou fine avale li pendant namaaz,eski namaaz casse 2.Si wudhu casse quand nou pe faire namaaz,nou cpv refaire wudhu sans qui nou cause avec personne ensuite continuer nou namaaz.Dans sa cas la comment bizin faire?eski nu bizin refaire Allah-u-Akbar et continuer dans position qui nou ti ete ou bien comment bizin faire. 3.si nou wudhu finne casse apres premier sajdah,eski nu bizin refaire premier sajdah ou bien nou continuer lors deuxieme sajdah.
